Our Services support early discharge from or prevent unnecessarily prolonged stays in hospital. We supply qualified nurses who are specialized in different fields and who have experience in general nursing as well as nurses who are able to deliver specialist nurse care like peg feeds, ventilators care, surgical care and end of life care.

Personalised Care Packages

Caritas Care Solutions can organise a complete Plan of Care for you. When you or a family member have a temporary or long-term physical or mental illness, are simply recovering from surgery or an illness, Caritas Care Solutions can design a programme to meet your specific needs & depending on your situation, we can provide services to regain independance and/or enhance quality of life.

Intermediate Care

Includes a range of short-term treatment or rehabilitative services designed to promote independence, maximize health and self-confidence and chances of living independently. The plan of care reduces the length of time you might be in hospital unnecessarily, or help you to avoid unnecessary admissions to hospital, support early discharge from or prevent unnecessarily prolonged stays in hospital following an ‘acute hospital admission’and prevent premature admission to long-term residential care.

Continuing Care

This package of care is for individuals who are not in hospital, but have complex ongoing healthcare needs. We supply qualified nurses who are specialized in different fields and who have experience in general nursing, as well as nurses who are able to deliver specialist nurse care like peg feeds, ventilator care, surgical care and end of life care.
